Role
As the Clinical Care Coordinator, you will take the lead in overseeing and coordinating clinical care, guiding a dedicated nursing team to deliver the highest standards of care—all with the strong backing of a supportive Care Manager and Facility Manager. 

Responsibilities 

• Provide clinical leadership, support, coaching and mentoring to staff
• Collaborate with other healthcare professionals to ensure the effective development and delivery of appropriate care plans
• Ensure compliance with the facility's documentation requirements
• Maintain clinical governance, supervise infection control protocols, and oversee medication management
• Foster strong relationships with residents, families, and stakeholders to promote optimal health outcomes  

• Tertiary qualification in Nursing with AHPRA Registration
• Full Australian working rights 
• Strong knowledge of regulatory requirements, care governance, and clinical standards in aged care
• Proven ability to lead teams and manage complex care scenarios in an aged care environment
• Demonstrated understanding of AN-ACC assessment processes
• Previous experience working within a Residential Aged Care setting in a similar role
