- Lead and manage the clinical care team to deliver person-centered care
- Develop, implement, and regularly review individual care plans
- Oversee daily clinical operations and ensure adherence to best practices
- Mentor and supervise nursing and clinical staff
- Monitor resident health and implement timely interventions
- Collaborate with other departments to provide holistic care
- Maintain accurate documentation and compliance with aged care standards
- Drive quality improvement initiatives and conduct audits
- Liaise with families and health professionals to ensure comprehensive care
- Support ongoing training and development of clinical staff
- Registered Nurse with current AHPRA registration (minimum 4 years’ experience)
- Previous experience as a Clinical Care Coordinator or similar senior clinical role
- In-depth knowledge of Aged Care Quality Standards and regulatory requirements
- Strong clinical assessment and decision-making skills
- Proven leadership capabilities and experience mentoring clinical teams
- Excellent communication and interpersonal skills
- Highly organised with strong attention to detail
